This conference will provide employers with information on how to best utilize Community Health Workers (CHWs) in meeting the future changes in health care delivery. CHWs are effective in meeting the “Triple Aim” of the Affordable Care Act: to increase accessibility of healthcare, improve cost-containment, and raise the quality of care. Topics to be discussed are: What are CHWs? What roles do CHWs play within provider settings and organizations? How are they trained? How are CHWs currently being used in Connecticut? How can CHWs be utilized in the State Innovation Model (SIM)? How are CHWs paid? How can CHWs be paid through sustainable payment? How do CHWs affect quality outcomes and cost effectives with healthcare reform? How are CHWs encouraged in the ACA?
This event is sponsored by Capital Community College, Connecticut Health Foundation, Gateway Community College, Housatonic Community College, Khmer Health Advocates, and the CT AHEC network.
$30.00 cost covers breakfast and a box lunch. Parking will be validated at Temple Street Garage.
