Help identify CHW’s or similarly titled individuals

Community health workers are known by many different names. The umbrella term “community health worker” (CHW) embraces a variety of community health aides selected, trained and working in the communities from which they come.

CT is one of many states that are establishing an association dedicated to enabling CHWs to take the lead in the community movement to organize, define and strengthen the profession of community health work.

Help us to identify CHW’s or similarly titled individuals…

WHEN: Starting on Monday July 14th, 2014, you may receive a phone call to gather information to help identify CHW’s and similarly titled individuals. Also, a survey will be available on Survey Monkey to help us in the identification process. You can help by either participating on the phone, or completing a survey. If you can get others to take the survey, it will be greatly appreciated.
