General Description:

The Southwestern Area Health Education Center (SWAHEC) is recruiting for 2 Community Health Workers to participate in achieving the deliverables for the Community Health Worker (CHW) initiative for Southwestern AHEC and the CT AHEC Network. One CHW will be stationed in Waterbury and Shelton; one CHW will be stationed in Hartford and New London.The CHW Initiative consists of: developing a system for the CHW workforce in Connecticut on a continuum that incorporates stakeholder engagement, infrastructure, policy and development of sustainability, education and training for CHWs, technical assistance and support for employers and supervisors, and community integration. The Community Health Workers will also conduct follow-up with the CHW interns participating in the Early Detection Program.

 

The programs and/or projects of the CHW initiative may be scientific, research, education, and/or services oriented, and are usually funded through contract/grant provisions.

 

Duties and Responsibilities:

Working under the supervision of the Southwestern AHEC Program Manager, the Community Health Worker responsibilities include:

  • Support engagement efforts of CHW stakeholders in discussions/planning meetings.
  • Assist in development and implementation of CHW Workforce Development Plan.
  • Support development/expansion of CHW Association.
  • Participate in regional CHW activities.
  • Identify model and best practice resources for CHWs and CHW employers.
  • Contribute to training curriculum for CHWs.
  • Support oversight of CHWs placed in provider sites.
  • Support development of policies, standards and evaluation protocols for CHWs.
  • Collect and compile community resources in their assigned geographic area of the state.
  • Identify and engage Community Health Workers in their regions.
  • Develop an inventory of potential and existing CHW workforce agencies.
  • Participate in organizing and implementing information sessions/meetings for regional stakeholders to promote opportunities for CHW employment.
  • Other relative duties as assigned by supervisor to support the CHW initiative.

 

Unusual Working Conditions:

 

Work hours generally take place within the normal work day. Occasionally, additional hours may be required to meet peak workloads and emergencies, and to handle special requirements of Southwestern AHEC and CT AHEC Network programs. Personal transportation necessary as local and state travel may be required.

 

Minimum Qualifications Required:

 

Work Experience and Educational Requirements:

 

A minimum of two years’ experience as a Community Health Worker.

A minimum of a High School diploma; an Associate’s Degree preferred.

CHW Core Competency training preferred.

· Knowledge, Skills, Ability: Basic knowledge, familiarity, and use of computer hardware and software (Microsoft Windows, Word and Excel). Multicultural and bilingual (English/Spanish) preferred. Strong interpersonal communication skills and writing skills are essential.

· Qualifications: Candidates must be trusted members of the communities they will be assigned to serve. Considerable knowledge and experience working with community-based organizations and in clinical settings is required. Training and experience working as a Community Health Worker is required. Associate’s degree is preferred. Own, reliable transportation is required. Ability to lift a minimum of 40 pounds is required.

· Work Schedule: This position is full-time, 40 hours per week.

 

 

Full Time Equivalent Salary: $40,000
