Town of Stratford Health Department Putting on AIRS Asthma Program
Per Diem Contractual RN, Respiratory Therapist, Certified Asthma Educator
NATURE OF WORK
Under the supervision of the Putting on AIRS Coordinator, the candidate will perform home visits with asthma patients per diem as needed for a State-funded asthma program. The candidate is responsible for:
- Accompanying an environmental sanitarian on Asthma visits scheduled in Bridgeport and Stratford with occasional visits scheduled in nearby municipalities within Fairfield County.
- Providing one-on-one asthma education to patients with uncontrolled asthma referred to the program.
- Completing all necessary paperwork and assisting with program follow-up and evaluation.
- Perform related work as deemed necessary by the Putting on AIRS Coordinator and Public Health Nurse.
Visits are typically scheduled by the Coordinator between 9:00 am and 5:00 pm with occasional later evening hours or weekends as determined by the Coordinator, Contractor, and Environmental Sanitarian.
Candidates should have a valid license in nursing (RN), respiratory therapy (RT) or Certified Asthma Educator. as well as experience in asthma education and management. Professional Liability/Malpractice insurance required.
