Healthy Connecticut 2025: State Health Improvement Plan (SHIP 2.0) is in the final stages of development with a target release date of early 2021. We are inviting you to provide feedback on the most current version of the plan, which outlines a strategy to improve health and racial equity in the state of Connecticut, specifically focusing on the root causes of health inequities (structural racism and inherent bias).
The development of the plan included cross-sector collaboration with state and local partners and community members, who we thank for their contributions during the entire SHIP planning process. Starting with the 2019 SHIP Coalition Summit, which brought together over 120 partners from across the state, and continuing with six regional community feedback sessions, and a series of virtual planning sessions to identify goals, objectives, and strategies, partner input has resulted in a plan that will guide health improvement efforts in the state over the next five years.
