Conference Evaluation

If you attended the conference in any capacity, please use this form to give us feedback about your experience so we can make next year's conference even better. Please complete the conference evaluation by the end of the day on Wednesday, November 1st.

Needs Assessment

Let us know what you love about CPHA and how we can make it even more responsive to your public health needs.  This survey is open to those who study, practice or teach public health or allied fields in Connecticut.  You don't need to be a member of CPHA or to have attended the conference.  By completing our survey in its entirety, you will be entered to win one of eight (8) $20 gift cards to Panera or Starbucks. Please note that current CPHA board members are welcome to complete the survey, but are ineligible for the gift card drawing. Please complete the Needs Assessment by November 15th at the latest.

CHES / MCHES Credits

Through the generosity of the Yale School of Public Health Office of Public Health Practice, CPHA is able to offer CHES / MCHES credits to those NCHEC certified individuals who attended our 2023 Annual Conference.  In order to receive credits, you must complete and submit the attached evaluation form absolutely no later than November 27, 2023.

Indigenous Scholars' Fund & Land Map

In the interest of diversifying the public health workforce and supporting native people, at the 2023 Annual Conference, CPHA launched its Indigenous Public Health Scholars' Fund to financially assist Indigenous Connecticut students pursuing undergraduate or graduate studies in public health.  Your donation can help finance this fund. Whether or not you chose to donate, please use this tool to learn more about the land you live, work and play on, and look at these suggestions for what you and your organization can do beyond just land acknowledgements.

2023 CPHA Board Elections

Current CPHA members, and only current CPHA members, are entitled to vote in CPHA's 2023 Board Elections.  Elections will take place between 5 pm on Wednesday, October 25th and 5 pm on Friday, October 27th 2023.
