2024 Annual Conference Evaluation & Booklet
If you attended CPHA's 2024 Annual Conference in any capacity (i.e., as a presenter, attendee, sponsor/ exhibitor, etc), please use this form to give us feedback about your experience so we can make next year's conference even better.
This evaluation has 15 short questions and we expect will take you about 4 minutes to complete.
As an incentive, we will give away a public health gift basket to two individuals who complete the survey and provide us with their contact information. Even if you don't want to share your contact info with us, you can still complete the survey; you just can't be entered to win the gift basket.
